All District Elementary Music Registration Form

School Year Tuition:

  • $399 (Includes music book, 26 one hour classes during the school year, 2 rehearsals, and 2 concerts). All string and band classes start directly after school. Scholarships and payment plans are available. See information below.The program tuition does not include an instrument. You can rent or purchase an instrument from any music store. Only one discount can be used for each registration. Discounts cannot be combined.

Jump Start Band & Strings or Level I Classes - Designed for kids with little or no experience
Level II Classes(Keyboard/Guitar/Mini Music)Designed for kids with previous experience or completion of Class I
Orchestra or Jump Start Jazz Band — Designed for kids with music experience (1 or more years). Jazz can be taken in conjunction with Band II Classes.


Choose one or two classes for your child to attend. For band and string classes, students can attend any school location or class even if they don't attend that school. Note that our keyboard and guitar classes are held at Applied Music Studio.

Emergency Information, Indemnity & Consent Release

INDEMNITY AND HOLD HARMLESS AGREEMENT

I/we hereby grant permission for my child to participate in the Los Alamitos Youth Center, Inc. program for which I am registering. I agree to indemnify and hold harmless the Los Alamitos Youth Center, City of Los Alamitos, Los Alamitos Unified School District, Rossmoor Community Services District, the Applied Music Studio and any other entity associated with the program, their officers, agents and employees from any liability, claim or action arising out of such participation. I understand that this program is not bound by the responsibilities and legalities that accompany a licensed daycare program. I further certify that my child is in good health and has no physical or other impediment which would endanger him/her or any other participant in taking part in such an activity.

CONSENT TO TREAT A MINOR

I certify that I am the parent or legal guardian of the child being enrolled in this program. I hereby authorize and consent to any x-ray examination, anesthetic, medical or surgical diagnosis rendered under the general or special supervision of any member of the medical staff and emergency room staff licensed under the provisions of the Medicine Practice Act or Dentist licensed under the provisions of the Dental Practice Act and on the staff of any acute general hospital holding a current license to operate a hospital from the State of California Department of Public Health. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care required but is given to provide authority and power to tender care which the aforementioned physician in the exercise of his/her best judgment many deem advisable. It is understood that effort will be made to contact the undersigned prior to rendering treatment to the patient, but that any of the above treatment will not be withheld if the undersigned cannot be reached. This authorization is given pursuant to the provisions of section 25.8 of the Civil Code, State of California.

This consent will remain in effect until rescinded in writing.

CONSENT FOR FIRST AID TREATMENT, TRANSPORTATION & PHOTOGRAPHIC RELEASE

I hereby authorize the staff of the Youth Center to provide immediate first aid to my child in the event of illness or injury. In addition, if this program provides for the transportation of my child, I hereby grant permission to the Youth Center to provide such transportation. Lastly, I hereby give the Los Alamitos Youth Center, Inc., it's successors and assigns, the absolute and irrevocable right and permission with respect to photographs, videos, motion pictures, and /or sound recordings being taken of my child: (a) to use, reuse, publish and republish in whole or in part and (b) to use my child's name. I further release the Youth Center from any claims and demands arising out of the use of same.



School Year Payment Plans:

SUMMER MUSIC Please add me to the mailing list for summer registration.


FLEXIBILITY: Children may choose to attend a class at any school if the class day and time is more convenient. The Youth Center does not provide transportation. Instructors are subject to change based on availability.

CLASS MINIMUM: Classes subject to a six (6) student minimum. If it is necessary to cancel a class as a result of insufficient enrollment, students may enroll at another school.

IMPORTANT REFUND POLICY: Students and parents have four (4) full class meetings in which to evaluate the program. A full refund of the program fee, less the $50 administration charge, will only be granted if written notice of withdrawal is received by The Youth Center office prior to the beginning of the 5th class meeting. Following the start of the 5th class, all program fees will be considered fully earned and no refunds, full or partial, will be granted.

PAYMENT SCHEDULE: If making payments, you will be automatically charged monthly on the 15th until the full program fee is paid. If payment is not made by the 30th of the month due, a $20 late fee will be assessed.

SCHEDULE: The School Year Music Program generally starts in September and ends in June (depending upon the school).

MAKEUPS: We do not provide makeups or refunds for missed classes. You may attend another school for an extra class if you need to miss your regular class time.
